Arden & GEM’s healthcare solutions team will be joining the NHS Population Health Management (PHM) Conference on 21 April 2022 to explore how ICSs can create a healthy culture which drives effective PHM.

The NHS Population Health Management Conference, hosted by Convenzis, will take place virtually on Thursday 21 April. The event aims to provide update and clarity on this developing approach and share best practice from across the NHS ecosystem.

The lunchtime session, at 12.55pm, will be delivered by Arden & GEM’s Associate Director for Leadership and OD, Ayesha Janjua, a system change practitioner who has worked with a number of local system leaders to develop their vision, strategy and culture for the successful delivery of population health management and outcomes-based commissioning.

Effective PHM is pivotal to the way health and care organisations work together to improve the health and wellbeing of their populations, a core strategic aim for all ICSs. In this session, Ayesha will discuss how ICSs can create a healthy culture which drives effective PHM, sharing her experience of supporting systems (and their component organisations) to address issues around culture, relationships and ways of working to drive population health approaches and develop effective skillsets that are crucial for an enabling and positive culture. Ayesha will also be exploring the concepts of ‘stewardship’ and value-based population health that are critical to the system-wide adoption of PHM.
