3. Optimise (first real stage) 27–34 Partners improve how existing resources work together; closing optimisation gaps becomes the system’s main task. Full mapping of all agency investments (finance, workforce, outcomes).
Longer-term contract commitments where feasible.
Outcomes-based commissioning frameworks introduced.
Simulation of optimisation gap closure (what joint working, prioritisation, accountability, contract forms and funding flows are needed).

4. Grow 35–42 Ambition increases; the system tests a live investable prototype underpinned by integrated frameworks and shared financial commitments. Integrated risk, performance and outcomes contracting framework in place.
Sinking fund/pooled reserve established to demonstrate shared commitment and de-risk investment.
First blended finance agreement and delivery model in development.
5. Scale & Spread 43–50 Organisation/system replicates the model across wider populations and places, with sustained investor confidence. Multiple investable cases in pipeline.
Outcomes-based contracts portfolio approach.
Regional Health Investment Framework embedded.
Ongoing evaluation and replication across sites.
